Application FormsApplication forms for Active or Associate membership in a unit may be obtained from that unit or from Dominion Headquarters. Applicants for a Unit membership must be sponsored by two (2) members. Application forms for Members-at-Large may be found by clicking here, then printing out the form, filling it out and mailing it into the Dominion Command Office in Ottawa. If you are a Member-at-Large, our newsletter will be mailed directly to your home. |
